ciSURVEY: provides a simple yet intuitive interface to create your very own survey. You can select a range of answer types like radio buttons, checkboxes, drop down lists or even a textfield. If you manage to create the questions in the wrong order, don't panic. ciSURVEY: gives you the capability to re-order the questions as you see fit.Another important component of the ciSURVEY: module is the ability to preview the survey before showing it to the world.
